Scaling and Polishing: Dentistry's Best Value Visit

If you only ever do one thing at a dentist, do this one. A scaling appointment costs a fraction of the treatments it prevents, and it is the single highest-return visit in dentistry.

It is also the treatment surrounded by the most persistent myth in Pakistani dentistry.

The myth, addressed directly

"Scaling loosens your teeth."

It does not. This belief is widespread and completely wrong, and it keeps people out of the chair until the damage is done.

Here is where it comes from. When tartar builds up heavily between and around teeth, it forms a rigid mass that acts like cement. Meanwhile, underneath it, bacteria are quietly destroying the bone and ligament that hold the tooth in place.

Remove the tartar and the teeth feel different, because the false splint is gone. The looseness was already there. The tartar was hiding it, not preventing it.

Leaving tartar in place to keep loose teeth feeling firm is like leaving rust on a bridge because it is holding the girders together.

What tartar actually is

Plaque is the soft film that forms on teeth within hours of brushing. Brushing removes it.

Left alone, plaque mineralises using calcium from your saliva and becomes tartar, which is hard, porous and bonded to the tooth. No toothbrush removes it. No mouthwash dissolves it. It comes off with an ultrasonic scaler or it does not come off.

The part that matters is what happens below the gum line, where you cannot see it and cannot reach it. That is where bacteria sit against bone and cause the slow, painless loss that ends with teeth genuinely loosening in your forties.

Why it is painless until it is not

Gum disease has almost no early symptoms. It does not hurt. That is precisely the problem.

The one signal most people get is bleeding when brushing. Almost everybody misinterprets it as a reason to brush that area more gently, which makes it worse.

Bleeding gums are inflamed gums, and inflammation is driven by plaque. The answer is to clean them more thoroughly, not less, and to have the tartar removed professionally.

What actually happens in the appointment

  1. Ultrasonic scaling. A vibrating tip breaks up hardened tartar above and below the gum line. It touches deposits, not enamel.
  2. Hand scaling below the gum. Where the damage actually happens.
  3. Polishing. A rotary paste lifts surface staining from tea, coffee, paan and tobacco.
  4. Fluoride. Strengthens enamel and reduces the mild sensitivity that follows a deep clean.

Forty-five minutes to an hour. Rarely uncomfortable. Local anaesthetic is available if your gums are badly inflamed.

Scaling versus whitening

A useful number of patients book whitening, have scaling done first as preparation, and then decide they no longer need the whitening.

Much of what people read as "yellow teeth" is surface staining sitting on top of tartar. Remove it and the teeth underneath are several shades brighter than expected.

Get scaled first. Then decide whether you still want whitening. The gel cannot penetrate tartar anyway, so whitening on unscaled teeth produces a patchy result at full price.

How often

  • Most people: every six months
  • Smokers and paan users: every three to four months
  • Diabetic patients: every three to four months, since gum disease and blood sugar control worsen each other
  • History of gum disease: as advised, usually every three months
  • Pregnancy: yes, and it is recommended rather than merely permitted

Afterwards

Mild sensitivity for a day or two is normal, especially where tartar had been insulating exposed root surfaces. It settles.

Avoid very hot, very cold and acidic food for 24 hours, use a soft brush for a few days, and skip smoking for at least a day so the gum tissue can settle.

Then book the next one before you leave. Six months is easy to forget and expensive to remember late.
